The Seafarer’s Identity Document (AIM) and the Seaman’s Book (CM) are issued to citizens of the Republic of Moldova and foreigners with the right of permanent residence on the territory of the country.

When submitting an application for the production and issuance of a seafarer’s identity card and seaman’s book at the headquarters of the Naval Agency of the Republic of Moldova, the applicant shall submit the following documents:

1. Identity card of the citizen of the Republic of Moldova or, if applicable, the permanent residence permit of the foreign citizen;

2. Passport of the citizen of the Republic of Moldova, the passport of the foreign citizen or, as the case may be, the valid travel document of the foreign citizen issued by the state where the person has established residence (with a validity period of at least 1 year);

3. Criminal record issued by the competent authority of the Republic of Moldova;

4. Certificates of competency and valid confirmations of certificates of competency (for seafarers/service personnel on board ships) or academic certificate (transcript) from the educational institution (for students);

5. Medical certificate form 086 (issued by the family doctor) or medical certificate issued by an authorized medical center (Seaman’s Medical Certificate);

6. 2 photos 3×4 cm, color, matte.
